Patient forms are emailed directly to you (from [email protected]) upon scheduling your initial visit. To save time when you get to the office for your treatment, please fill out your patient forms completely before your appointment. Acupuncture intake forms tend to be more detailed than the typical medical form you may be accustomed to so give yourself plenty of time to complete them. Once you have completed your intake forms for your initial appointment, you simply have to bring yourself and a smile to the office moving forward!
Please arrive 10 minutes before the start of your appointment so I may review your forms before we begin and have you fill in any areas that were possibly missed or need more details. This way we can make the most of our time together to give you the best possible experience and treatment.
